• Call 0800 012 2008 to request an application pack
• Email sales@patientchoice.org to request an application pack
  (Please give your name, address, contact telephone number, level of cover needed i.e. single, couple, family and the date of birth of the eldest person requiring cover)

Please note PatientChoice is only available if you are a resident in the UK for a minimum of 180 days a year
